e54b27b967 arch: define struct arch_esf and deprecate z_arch_esf_t
Make `struct arch_esf` compulsory for all architectures by
declaring it in the `arch_interface.h` header.

After this commit, the named struct `z_arch_esf_t` is only used
internally to generate offsets, and is slated to be removed
from the `arch_interface.h` header in the future.

bbe5e1e6eb build: namespace the generated headers with zephyr/
Namespaced the generated headers with `zephyr` to prevent
potential conflict with other headers.

Introduce a temporary Kconfig `LEGACY_GENERATED_INCLUDE_PATH`
that is enabled by default. This allows the developers to
continue the use of the old include paths for the time being
until it is deprecated and eventually removed. The Kconfig will
generate a build-time warning message, similar to the
`CONFIG_TIMER_RANDOM_GENERATOR`.

Updated the includes path of in-tree sources accordingly.

Most of the changes here are scripted, check the PR for more
info.

7c80746bc6 tests: kernel: use TC_END_REPORT for faults
Use TC_END_REPORT() to report "PROJECT EXECUTION FAILED"
instead of a call to printk() for ZTEST enabled tests. as
this prints the failed message as well as the RunID (and

fb38b90481 tests kernel: Exclude posix arch for userspace tests
These tests cannot be run in this architecture as it does not
support userspace.
Today they are filtered by kconfig, which works but spends
time running cmake.
As native_posix is a default test platform it is better
to filter it alltogether by arch, which saves quite a lot
of time.

Twister now supports using YAML lists for all fields that were written
as space-separated lists. Used twister_to_list.py script. Some artifacts
on string length are due to how ruamel dumps content.

116998c677 tests: kernel: print FAILED when wrong faults caught
For some kernel tests, faults and exceptions are expected.
They are caught and the test would continue if the reasons
for faults are as expected. However, when the unexpected
reasons are encountered, the code simply prints a message
and calls k_fatal_halt(). When running under twister,
these messages are not the expected failed messages so
twister will spin till timeout although the execution
has already been halted. This adds another printk() before
halt to signal twister that the test has failed and bails
early.

6d4e3dd611 tests: dynamic_thread: fix test_thread_index_management
This test was working by accident onarm64 and riscv64. Those
architectures have large register files, even more so considering
their 64-bit nature.

This test works by calling k_object_alloc(K_OBJ_THREAD) until thread
index exhaustion. However here it exhausted heap memory before running
out of thread indexes. There was a test to make sure that wasn't the
case by attempting a k_malloc(256). But here that succeeded just
because 256 is far smaller than a struct k_thread on the above
architectures.

Fix this by:

- attempting an additional allocation with the actual object size
  instead of an arbitrary 256 bites
- increasing the heap size as 8192 was clearly insufficient for the
  above platforms.

1cccc8a8fe cmake: increase minimal required version to 3.20.0
Move to CMake 3.20.0.

At the Toolchain WG it was decided to move to CMake 3.20.0.

The main reason for increasing CMake version is better toolchain
support.

Better toolchain support is added in the following CMake versions:
- armclang, CMake 3.15
- Intel oneAPI, CMake 3.20
- IAR, CMake 3.15 and 3.20

b76cbe9af9 test: dynamic_thread: Fix race condition
There is a race between k_sem_take() and k_object_access_grant() so it
is possible (especially when testing SMP) that the thread tries to take
the semaphore before the originating thread has had the chance to
grant it permission.

This makes a difference when building with CMake and forgetting
ZEPHYR_BASE or not registering Zephyr in the CMake package registry.

In this particular case, REQUIRED turns this harmless looking log

407b49b35c cmake: use find_package to locate Zephyr
Using find_package to locate Zephyr.

Old behavior was to use $ENV{ZEPHYR_BASE} for inclusion of boiler plate
code.

Whenever an automatic run of CMake happend by the build system / IDE
then it was required that ZEPHYR_BASE was defined.
Using ZEPHYR_BASE only to locate the Zephyr package allows CMake to
cache the base variable and thus allowing subsequent invocation even
if ZEPHYR_BASE is not set in the environment.

It also removes the risk of strange build results if a user switchs
between different Zephyr based project folders and forgetting to reset
ZEPHYR_BASE before running ninja / make.

f2422f1f19 tests: clean up fatal error handlers
- k_sys_fatal_error_handler() can return on all platforms,
  indicating that the faulting thread should be aborted.
- Hang the system for unexpected faults instead of trying
  to keep going, we have no idea whether the system is even
  runnable.

Prevents infinite crash loops during tests.

71ce8ceb18 kernel: consolidate error handling code
* z_NanoFatalErrorHandler() is now moved to common kernel code
  and renamed z_fatal_error(). Arches dump arch-specific info
  before calling.
* z_SysFatalErrorHandler() is now moved to common kernel code
  and renamed k_sys_fatal_error_handler(). It is now much simpler;
  the default policy is simply to lock interrupts and halt the system.
  If an implementation of this function returns, then the currently
  running thread is aborted.
* New arch-specific APIs introduced:
  - z_arch_system_halt() simply powers off or halts the system.
* We now have a standard set of fatal exception reason codes,
  namespaced under K_ERR_*
* CONFIG_SIMPLE_FATAL_ERROR_HANDLER deleted
* LOG_PANIC() calls moved to k_sys_fatal_error_handler()

Address a coverage gap in kernel/userspace.
Unfortunately, in the process of fixing this, a bug was
discovered, see #17023.

This test is user mode specific, filter the testcase
on whether userspace is available instead of ifdefing
the code.

3ae52624ff license: cleanup: add SPDX Apache-2.0 license identifier
Update the files which contain no license information with the
'Apache-2.0' SPDX license identifier.  Many source files in the tree are
missing licensing information, which makes it harder for compliance
tools to determine the correct license.

By default all files without license information are under the default
license of Zephyr, which is Apache version 2.

7b1ee5cf13 tests: CONFIG_TEST_USERSPACE now off by default
Unlike CONFIG_HW_STACK_PROTECTION, which greatly helps
expose stack overflows in test code, activating
userspace without putting threads in user mode is of
very limited value.

Now CONFIG_TEST_USERSPACE is off by default. Any test
which puts threads in user mode will need to set
CONFIG_TEST_USERSPACE.

This should greatly increase sanitycheck build times
as there is non-trivial build time overhead to
enabling this feature. This also allows some tests
which failed the build on RAM-constrained platforms
to compile properly.

tests/drivers/build_all is a special case; it doesn't
put threads in user mode, but we want to ensure all
the syscall handlers compile properly.

7eabab2f5d samples, tests: Use semi-accurate project names
When using an IDE (e.g. Eclipse, Qt Creator), the project name gets
displayed. This greatly simplifies the navigation between projects when
having many of them open at the same time. Naming every project "NONE"
defeats this functionality.

This patch tries to use sensible project names while not duplicating
too much of what is already represented in the path. This is done by
using the name of the directory the relevant CMakeLists.txt file is
stored in. To ensure unique project names in the samples (and again, in
the tests folder) folder, small manual adjustments have been done.

55ee53ce91 cmake: Prepend 'cmake_minimum_required()' into 'app' build scripts
Prepend the text 'cmake_minimum_required(VERSION 3.8.2)' into the
application and test build scripts.

Modern versions of CMake will spam users with a deprecation warning
when the toplevel CMakeLists.txt does not specify a CMake
version. This is documented in bug #8355.

To resolve this we include a cmake_minimum_required() line into the
toplevel build scripts. Additionally, cmake_minimum_required is
invoked from within boilerplate.cmake. The highest version will be
enforced.

This patch allows us to afterwards change CMake policy CMP000 from OLD
to NEW which in turn finally rids us of the verbose warning.

The extra boilerplate is considered more acceptable than the verbosity
of the CMP0000 policy.
