b83b9bede3 Bluetooth: ATT: call-back on buffer destroy
This is just as arbitrary as what was before, but simpler.

Before this change, the callback were invoked upon receiving the num
complete packets event.
This did not necessarily work with all spec-compliant controllers.

Now the callback is invoked as soon as the lower layer destroys the
buffer. ATT shouldn't care whether L2CAP sends it over RFC1149 or
something else after that point.

ddf172c187 Bluetooth: gatt: Fix automatic resubscription causing CCC removal
This fixes CCC subscriptions that were removed if the automatic
resubscription was aborted by ACL disconnection.
As the client renews subscriptions, there is no point of removing those
if the link is disconnected unexpectedly.
The API user won't be notified about the failure, as the automatic
resubscriptions are implicit, and after reconnection the subscriptions
will be still valid.

9f42c92e26 Bluetooth: Host: Fix GATT Long Read for EATT
When EATT is established, the value returned from `bt_att_get_mtu` is
not useful to determine the ATT_MTU that applies to a ATT response. This
is because `bt_att_get_mtu` may return the value for a different bearer
than the request is serviced on.

To fix this, the params struct for the GATT read operation is given a
new field that will record the ATT_MTU that applies to this ATT
operation. This value is then used to determine if the GATT long read
operation is concluded.

d4d0956516 Bluetooth: ATT: Split BT_ATT_MTU for UATT/EATT
This is a refactor. There is no behavioral change because
`BT_LOCAL_ATT_MTU_UATT` is `BT_LOCAL_ATT_MTU_EATT + 2` is
`BT_ATT_BUF_SIZE` is the old `BT_ATT_MTU`.

The old `BT_ATT_MTU` was wrong for EATT bearers. EATT MTU is two bytes
less because of ECRED overhead.

Instead of the old `BT_ATT_MTU`, we define one for each bearer type. We
also define the max of them to use as a convenience for allocating
buffers that fit either.

To avoid confusion, 'LOCAL' has been added to the name. This is to
differentiate it from what the spec calls 'ATT MTU', which is a
negotiated property. (It is the minimum of the two side's local ATT
MTU.)

ccaa7b291e Bluetooth: Host: Check if channel option is valid
Disallow trying to send requests or notifications on only EATT channels
if no EATT channels are connected or the link is not encrypted. In these
cases the operation will always fail, so it should not be queued.

This allows the application to choose wether Unenhanced ATT, Enhanced
ATT, or Any ATT channel shall be used for the
request/command/indication/notification.

ffefce7745 Bluetooth: host: Rewrite bt_gatt_notify_multiple
This API now becomes a low-level API, in the sense that it should only be
used if the app developer knows what he is doing and wants to ensure a
ATT_MULTIPLE_HANDLE_VALUE_NTF PDU goes on-air.

For the other 99% of use cases, `bt_gatt_notify` should instead be used, as
it will automatically upgrade to ATT_MULTIPLE_HANDLE_VALUE_NTF when
possible.

One can disable the batching of notifications when using `bt_gatt_notify`
by setting CONFIG_BT_GATT_NOTIFY_MULTIPLE_FLUSH_MS=0 .

This API doesn't support lookup by UUID any more.

7e9f652ee6 Bluetooth: host: Invoke GATT notify callback for each handle
In the case a call to `bt_gatt_notify_cb` gets its attribute data
batched with others to form an ATT_MULTIPLE_HANDLE_VALUE_NTF PDU, the
application developer would still expect one callback per API call.

c8b2279f89 Bluetooth: Host: Make tx complete callback work for EATT channels
Previously, if a callback was set for notifications, they would always be
sent over unenhanced ATT.

The nfy_mult_data was bigger than the buffer user_data and we were this
overflowing the buffer when setting the tx callback user_data. Now the
data is stored separately and only a pointer to it is added to the
buffer user_data.

e97413057a Bluetooth: Host: Fix type mismatch in bt_gatt_cancel
`bt_gatt_cancel` takes a `bt_gatt_.._params` pointer. The `params`
pointer is mistakenly passed on to `bt_att_req_cancel`, which expects a
`bt_att_req`.

This change makes bt_gatt_cancel locate the `req` and pass that to
`bt_att_req_cancel`.

6483e12a8a Bluetooth: Refactor bluetooth buffer configuration for simplification
Refactor and simplify the bluetooth buffer configurations to improve the
easy of configurations and eliminate invalid ones.
By moving configurations out of host and controller specific
configurations and into a common one it becomes easier to configure
the host and controller separately as the same configurations can be
used as would be for a combined build.

All HCI configurations are now given exluding the matching HCI header,
which eases the configuration as the application don't have to know the
different header sizes.
The BT_RX_BUF_LEN is split into ACL and Event, as well as the suprising
use of Command size.
BT_L2CAP_RX_MTU is removed as the stack does not support reassembling of
HCI ACL data to larger L2CAP PDUs. The application will have to set
ACL RX size and account for the L2CAP PDU header itself.
BT_EATT_RX_MTU was removed as it is only used for setting a different
default value for another option which leads to the stuck kconfig symbol
problem.

The configurations can be updated according to the table below:

Fixed invalid configurations setting either BT_L2CAP_RX_MTU or
BT_CTLR_DATA_LENGTH_MAX larger than BT_RX_BUF_LEN could lead to buffer
overruns.

Fix advertising report max data length calculation.
This always used the BT_DISCARDABLE_BUF_SIZE macro but this feature
can be turned off and advertising reports will be allocated from the RX
buffer in that case. Also controller-build does not have this buffer
(in hci_raw.c). Also the wrong HCI header was used in the calculation,
HCI event header should have been used instead of HCI ACL header.

10841b9a14 Bluetooth: host: Release ATT request buffers once sent
The ATT request buffers are held until the ATT response has been
received. This means that the ATT request buffers are released by the
RX thread, instead of the from the RX priority context of
num_complete.
This can cause a deadlock in the RX thread when we allocate buffers
and all the available buffers are ATT requests, since the RX thread is
the only thread that can release buffers.

Release the ATT request buffers once they have been sent and instead
handle ATT request resending by reconstructing the buffer from the
GATT parameters.

Also re-order the order of resource allocation by allocating the
request context before the buffer. This ensures that we cannot
allocate more buffers for ATT requests than there are ATT requests.

Fixed a buf reference leak that could occur when the ATT request buffer
has been allocated, but GATT returns an error before handing the
responsebility of the buffer to ATT, for example when bt_att_req_alloc
fails.
This is fixed by moving the functionality of att_req_destroy to
bt_att_req_free.

1d66c1e491 Bluetooth: host: Use CONFIG_BT_L2CAP_RX_MTU always to set L2CAP RX MTU
Remove dependency on CONFIG_BT_ACL_FLOW_CONTROL and use
CONFIG_BT_L2CAP_RX_MTU always to set L2CAP RX MTU.
The ATT MTU is set from two different KConfig options depending
on if CONFIG_BT_ACL_FLOW_CONTROL is enabled, which makes it
a confusing option and hard to provide a conf file that
supports multiple board configurations.

This changes the behavior when CONFIG_BT_ACL_FLOW_CONTROL and
CONFIG_BT_BUF_RX_LEN was used to set the L2CAP RX MTU, and by
extension the ATT MTU.

This adds support for EATT bearer which was introduced in 5.2, they work
as extra channels to have GATT traffic, at the moment it is completely
transparent to application when they are in use since the allocation
happens automatically.

d858264d9e Bluetooth: host: Convert bluetooth host to using k_timeout_t struct
Convert bluetooth host to using k_timeout_struct for the timeout values.
This is mostly replacing s32_t with k_timeout_t.
In l2cap the handling of no timeout in send channel request was removed
since the timeout is both documented as minimum of 1 second and never
given any no timeout value.

a779705f16 Bluetooth: GATT: Add bt_gatt_write_response_cb
This adds bt_gatt_write_response_cb works similarly to
bt_gatt_notify_cb which can take a callback to be called when the PDU
is considered transmitted over the air.

Note: This can also be used to disable the ATT flow control which would
blocks sending multiple commands without wainting their transmissions.

2975ca0754 Bluetooth: Kconfig: Rename CONFIG_BLUETOOTH_* to CONFIG_BT_*
The API name space for Bluetooth is bt_* and BT_* so it makes sense to
align the Kconfig name space with this. The additional benefit is that
this also makes the names shorter. It is also in line with what Linux
uses for Bluetooth Kconfig entries.

Some Bluetooth-related Networking Kconfig defines are renamed as well
in order to be consistent, such as NET_L2_BLUETOOTH.

ac74d8b652 license: Replace Apache boilerplate with SPDX tag
Replace the existing Apache 2.0 boilerplate header with an SPDX tag
throughout the zephyr code tree. This patch was generated via a
script run over the master branch.

Also updated doc/porting/application.rst that had a dependency on
line numbers in a literal include.

Manually updated subsys/logging/sys_log.c that had a malformed
header in the original file.  Also cleanup several cases that already
had a SPDX tag and we either got a duplicate or missed updating.

767c92e176 Bluetooth: Consolidate most outgoing ACL TX buffers into a single pool
Having TX buffers split into numerous pools has the downside of
increased memory consumption. This patch takes the initial step to
consolidate these pools into a single one, saving about 248 bytes of
RAM for a basic configuration.
