We faithfully emulate the GUS instrument mappings bug, which
can sound unpleasant. gusgonna.wad is now available as a
workaround.

OPL "Operating System support" section was mistakenly listed as a
level-1 heading, and heading levels in general were pretty screwed
up. Change them to saner nested levels.
